Following the highly publicized legal case that left a pharmacist charged with involuntary manslaughter, and despite advances in technology to improve medication safety, the world of health care is still grappling with how to reduce and eliminate medication errors. In this session, Chuck DiTrapano will address the importance of access to information and the barriers to making informed clinical decisions. Joining Chuck, Eric Cropp will discuss his personal experience of how a devastating medication error impacted his life and how he is raising awareness about the “second victim” of medication errors.
