Three significant opportunities in the home parenteral nutrition (HPN) transition, which may improve safety and customer satisfaction, will be reviewed in a case-based presentation. These include evaluation of the patient and/or caregiver’s preparedness to manage HPN; pre-discharge and in home education applying the teach back method for PN administration, self-monitoring, triaging common challenges in the home, and when to contact a clinician; and development of a safe HPN prescription, with critical evaluation of compatibility, stability, PN additives including those administered by the patient, and other intravenous medications and fluids. Each of these opportunities must engage the patient and/or caregiver and the inpatient and home infusion interprofessional teams in a collaborative handoff to optimize outcomes and mitigate complications for both the short and long term.
