Integrating Home Infusion Care Following Autologous Hematopoietic Stem Cell Transplantation: Clinical Trial to a Standard of Care

Innovative home infusion pharmacy collaboration with the bone marrow transplant team at the University of Kansas Health System, where patients are stratified to receive their 14 days post stem cell transplant treatment in their homes or alternative living location, versus the standard of care (clinic setting). Registered nurses seeing patients in the transplant at home program will be operating as home infusion field nurses for home visit activities. They will remain accountable to and part of the cancer care BMT program with dotted line reporting to home infusion. Patients will be seen by a nurse practitioner for evaluation each morning, managed as an outreach encounter. Based on the assessment and labs, any required treatments will be ordered through home infusion and administered in the afternoon by the BMT-RN under this program. The intravenous medication and supplies needed for administration will be dispensed from the home infusion pharmacy.

  • Discuss the evolution of the post HCT patient care environment.
  • Evaluate current research and literature to assess advancement to the home care model for the post HCT patient and caregiver.
  • Describe methods for integration and implementation of a clinical trial in your home infusion practice.
  • Review key interdisciplinary members in both BMT and Home Infusion settings to be involved to implement and deploy.
  • Identify barriers to implementing post HCT patient care in home infusion practice and describe strategies to overcome these challenges.
