Activity Description : The session would discuss collaborative strategies for home infusion providers to partner with health systems to optimize their infusion care continuum. We will cover referral pathways, approaches to engaging with hospital-based infusion departments and clinics, and how to articulate home infusion benefits to health-system leaders in a meaningful way. This topic will be valuable to the audience by outlining the versatility of a home infusion pharmacy in supporting system-level infusion strategy for key referral sources (i.e. health systems).
Handouts/Tools: Resources will include slides, workflow/referral diagrams, a case study, and interactive question/answer polling opportunities for the audience.
