Conducting Root Cause Analysis and Corrective Action Plan

This presentation will review how to conduct effective Root Cause Analysis (RCA) and implement Corrective and Preventative Actions (CAPA) in a home infusion setting. It will provide practical tools and resources to identify system-level failures, develop sustainable solutions, and foster a culture of continuous improvement. Additionally, the session will cover how to recognize and support healthcare workers experiencing Second Victim Syndrome (SVS), including strategies for institutional response and peer support.
