Education Track Session

Closing the Loop: Turning Performance Improvement into Real Clinical Outcomes

Activity Description : Performance Improvement (PI) generates meaningful insights, but gaps often occur in how those findings are communicated, understood, and applied across nursing and the interprofessional team in home infusion. When PI results are not clearly translated into shared expectations or coordinated action, teams may struggle to implement changes that strengthen safety, consistency, and patient outcomes in the home setting. This session addresses these practice gaps by enhancing the team’s ability to interpret PI findings, communicate them effectively, and participate in collaborative action‑planning that leads to measurable improvement. Participants will explore how PI information flows from leadership to frontline clinicians, identify common breakdowns in communication and accountability, and apply practical strategies to ensure PI results drive real, sustained changes in performance and patient outcomes.
