10. The Power of Partnership for Safe PN Transition of Care

Safe transition of care (TOC) from hospital to home on PN requires clinician expertise and collaboration between inpatient and home infusion nutrition support teams (HNST). There are many components of the PN regimen that have the potential to be missed during transition, which can be avoided if communication between teams takes place to review and adjust the PN regimen as appropriate during the TOC process. This roundtable discussion will emphasize points of communication between inpatients and HNST during the TOC process and stress the importance of utilizing published industry resources, such as clinical guidelines, consensus recommendations, and standardized PN order templates, to facilitate safe PN TOC.
