Based on our data for 2025, I will review the top 10 or so deficiencies noted from the 2025 ACHC pharmacy accreditation surveys (these surveys include sterile compounding pharmacies, specialty pharmacies, PCAB surveys). I will be sure to include only findings that would be pertinent to sterile compounding accreditations.
