06. ACHC Pharmacy Accreditation: Frequent Deficiencies in Pharmacies

Based on our data for 2025, I will review the top 10 or so deficiencies noted from the 2025 ACHC pharmacy accreditation surveys (these surveys include sterile compounding pharmacies, specialty pharmacies, PCAB surveys). I will be sure to include only findings that would be pertinent to sterile compounding accreditations.

Learning Objectives

Demonstrate an understanding of the most common deficiencies found during accreditation surveys and some tips for compliance.

Identify areas of strength and/or weakness found within their own organization in order to comply with accreditation standards.

Conduct a review of processes to prepare for any accreditation.
